Season 4 of The Boys has delivered one exceptional episode after another. Episode 6, titled Dirty Business, follows the eponymous group as they break into Tek Night‘s mansion. They learn about Homelander and Sister Sage’s secret plan and encounter the purported smartest individual on the planet. Sister Sage gets shot and is left so severely brain damaged that she fails to help Homelander present his plans to the influencers of the US government. This has made the viewers wonder whether Sister Sage’s brain damage is permanent, and if she has lost her powers.

No, Sister Sage likely has not lost her powers with permanent brain damage in The Boys Season 4.
In the fourth episode, she explains to the Deep that her brain hasn’t stopped growing, unlike most people. It constantly regenerates and is likely the cause of her superhuman intelligence, as per The Wrap. She also reveals that she can survive severe injuries to her brain and even asks him to give her a frontal lobotomy to lower her intelligence so she doesn’t have to be herself for a few hours. As seen in Episode 3, it’s something she even does to herself.
From what Sister Sage explains, if she is attacked on other parts of her body, the damage will be long-term or even permanent. But that’s not the case with her brain. So, it’s safe to assume that she hasn’t lost her powers after Marvin shot her.
